New Onsite System Registration Forms

As noted in the previous newsletter, all forms have been deleted from the Appendices of Title 124. Submittal requirements are now listed in the body of the regulations or you may use a form provided by NDEE. Included with this newsletter are NDEE registration forms for your use. Please note that these registration forms are system specific and include:
  • Septic System
  • Holding Tank
  • Household Domestic Lagoon
  • Title 124 Permit System
Please get rid of any old registration forms. Please copy the attached forms for your use. You may use these forms or one of your own as long as it meets the requirements found in Chapter 22 of Title 124 (effective date August 11, 2012). The forms are available on the NDEE website or printed copies will be provided on request.

Also included in this newsletter are the system registration instructions. Please take a moment to review the instructions and the new registration forms. Registration forms will be returned if not all required information is provided. Note that the old forms do not have all the required information.

The system registration fee remains at $140 for the registration of a system within 45 days of completion of construction, modification, reconstruction or alteration. Late fees still apply – initial late fee of $150 for a registration 46 to 90 days after completion ($140+$150=$290 total) or final late fee of $450 if registered 91 or more days after completion ($140+$450=$590 total).

As a reminder, for the purposes of registering an onsite system the Department has interpreted “completion of construction” to mean that all onsite wastewater treatment system components have been installed, constructed or modified so that the system piping, treatment devices or other appurtenances that convey, treat or dispose of wastewater are ready to be used. And as a reminder, the Installer is responsible for registering an onsite wastewater treatment system installed under a construction permit.

Did You Get Your Title 124?

A printed copy of Title 124 (effective August 11, 2012) has been mailed to you. If you did not receive your copy or need more copies please contact us. Title 124 can also be viewed or downloaded from the NDEE website.
